Superior mesenteric ischemia in a prisoner and its approach in forensic autopsy

In Costa Rica, the Regulation of Hospital and Forensic Autopsies establishes the mandatory performance of a forensic autopsy for any person deprived of liberty who dies. Although most deaths in this group are of natural origin, acute mesenteric ischemia is a rare and challenging cause to diagnose due to its nonspecific clinical presentation. This article analyzes the pathophysiology of a natural death caused by acute mesenteric ischemia in a 59-year-old inmate, resulting from the embolization of an intracardiac thrombus. It highlights the importance of forensic pathologists being familiar with and considering this condition during forensic autopsies to ensure an accurate diagnosis and clarify the circumstances surrounding the death.
